I am writing to formally request that [Local Authority Name] Local Authority carries out an Education, Health and Care Needs Assessment (EHCNA) for my child, [Child Name], who was born on [Date of Birth] and currently attends [School Name].
[Child Name] has significant and persistent difficulties that are adversely affecting their educational development and participation. Despite the school implementing SEN Support, [Child Name]'s needs have not been adequately met through current provision, and a more comprehensive assessment is required to understand and support those needs fully.
I believe [Child Name] may have special educational needs within the meaning of Section 20 of the Children and Families Act 2014, and that it may be necessary for special educational provision to be made in accordance with an Education, Health and Care Plan.
Under Section 36 of the Children and Families Act 2014, a parent has the right to request that a local authority carry out an EHC needs assessment. I am formally exercising that right through this letter.
I understand that [Local Authority Name] Local Authority must decide within six weeks of receiving this request whether to carry out an assessment. If you decide not to assess, I understand I have the right of appeal to the First-tier Tribunal (SEND).
